Application to Quarantine

Request to Quarantine – a document written by the MDAR that requests approval to quarantine at a specific facility. Both the rescue and the quarantine must complete separate parts. The rescue completes their part first and then sends it to the quarantine facility for them to complete their section and then forward to the MDAR for approval.

30.04(5) An Applicant may enter into a written agreement with the operator of an approved Isolation Room to perform the activities required by 330 CMR 30.08. Such written agreement shall be approved by the Department prior to the issuance of an Import License.

The MDAR has stated that they will only approved two or three isolation agreements per rescue. In addition, while most agreements are approved promptly, there have been times when it can take months to obtain approval. Rescues should ensure that they are in good standing with the State Attorney General Office.
